Learning outcomes
The student tailors communication to the individual care recipient and their close one(s). In doing so, she applies relevant conversation techniques. They analyse and reflect on their own communication and that of the care recipients and their relatives, and adapt their own communication accordingly:
The student communicates and collaborates with their own professional group and collaborators, positions themselves as a nurse and is open to perspectives of other care providers, the care recipient and their relatives for the benefit of continuity of care. The student applies appropriate communication methods and methods of reporting and transfer:
Expresses herself both orally and in writing in correct Dutch and uses appropriate professional jargon.
The student knows the latest ICT possibilities and applies these in addition to personal contact with care recipients and in cooperation with other disciplines. This takes into account laws and regulations. The student is aware of the limitations and possible effects of the application of ICT in healthcare:

- applies conversation techniques based on relevant literature.
- Translates and formulates professional jargon appropriate for the care recipient and vice-versa.
- Deploys appropriate aids if there is a limitation (e.g. deafness) and/or bottleneck (e.g. strong emotions or resistance) in the communication.
- In communication, the student takes intercultural aspects and non-native speakers into account.
- Advises, motivates and instructs the care recipients and their relatives and acts as a point of contact and refers to other care providers where necessary.
- Reflects on own communication.
- Writes and speaks in correct Dutch.
- Takes into account the different perspectives in the cooperation with other professionals, the care recipient, his/her nearest(s), volunteers and informal carers in the context, showing an equal attitude.
- Focuses on the care recipient.
- Applies methods for effective and efficient communication and reporting of consultation (multidisciplinary) and transfer, both orally and in writing, taking into account relevant laws and regulations.
- Identifies roles, expertise and competences of collaborators in the various care domains in their own region.
- Positions herself as a nurse in collaboration with collaborators.
- Formulates her vision of collaboration.

- Displays an open and inquisitive attitude towards ICT innovations in healthcare.
- Within his/her own field of work, investigates how to deal with the role of various social media, attunes this with the individual care recipient and makes the connection with his/her own professional role.
- Investigates in his/her own context which ICT innovations are relevant (e.g. care at a distance (e-health), domotics, care robots).

Content
You look for a care recipient who meets the characteristics of mid-complex level. You identify patient problems using the core set. You then talk to the care recipient (or, if applicable, his/her legal representative) to discuss which patient problems need to be addressed in a care coordination meeting, such as an MDO. But it may also be another form of consultation, as long as there is one or more participating disciplines, in addition to you as a nurse. So it depends on your context, how this assignment takes shape. You plan and organise this moment in which you coordinate care, depending on the working method within your organisation.
The aim is: coordination and continuity of care and your role as a nurse in this.
Processing/implementation:
You organise and participate in a consultation in which you bring the agreed patient problems (from the orientation phase) into your role as a nurse, taking into account your competences and expertise. Your work supervisor is present for appreciation.
You discuss this consultation directly with your work supervisor. If this is not possible, you can for instance record the consultation on video/audio (taking into account the laws and regulations regarding the use of ICT and privacy) and discuss it at a later time. You report the outcomes of the consultation according to the applicable guidelines and agreements in your context and feedback this to the care recipient and your team. If the care recipient is present at the consultation moment, you discuss the consultation with the care recipient afterwards.
Through self-study and exchange during the study days in the working group, you will deepen your knowledge of the theoretical backgrounds. You learn additional interview techniques for conversations with care recipients and their relatives and communication with other disciplines. And during peer review meetings, you reflect together with fellow students on cases you have experienced.
To show how you have implemented the learning outcomes, you may choose either a report or a presentation as form of assessment. It is compulsory to submit a report for at least one of the two learning tasks (HVMB18AVZM3 and HVMB18VARM3).
For the Fastswitch, the portfolio is the test form.
