The student acts on the basis of his/her own nursing expertise and responsibility and cooperates on the basis of equality with his/her own and other disciplines within and outside the health care sector with regard to (multidisciplinary) care and treatment goals in highly complex situations.
The student works as an autonomous professional, both within and outside their own organisation, together with other professionals or agencies to improve the quality and continuity of care.

Learning outcomes:
5.1.1 The student applies the principles of Shared Decision-Making to support care recipients in decision-making.

Content

The 'Shared Decision-Making' test consists of two parts. The first part of the test consists of an assessment of the performance assessment 'Shared Decision-Making' and a reflection report on the application of the principles of Shared Decision-Making. The second part of the test consists of an assessment by the work supervisor of functioning in the workplace in relation to the application of the principles of Shared Decision-Making, and the way in which, in cooperation with care recipients, their relatives and other disciplines, the continuity and quality of care within the chain is accounted for.

During the working group meetings, Patient Empowerment and the associated principles of Shared Decision-Making take centre stage. The principles of Shared Decision-Making are practised using case histories. Attention is also paid to how the continuity and quality of care can be justified in the cooperation with the care recipients, their relatives and other care professionals. The methodologies and tools used for this in practice are compared with each other and with theory.
The lesson also focuses on reflecting on one's own actions using various models and methodologies. The aim is to develop a critical and analytical attitude towards one's own (nursing) behaviour and that of colleagues and other disciplines.

At the Fastswitch, the portfolio is the form of assessment
