The student:
The module is offered in main phase 3: professionalisation (CBN, BN2020)
- describes the relationship between the concepts of self-management and co-reliance, describes how these concepts are applied in nursing and multi-disciplinary practice and describes their own views regarding these concepts . (1.3.1, 1.3.2, 1.3.3);
- conducts a (family) system-oriented assessment in conversation with a care recipient and one or more family members using a validated methodology and appropriate communication skills. (1.3.5, 1.3.6, 1.3.8, 5.2.10);
- conducts a (family) system-oriented conversation with care recipients and one or more family members based on a validated methodology and appropriate communicative skills . (1.3.6, 1.3.7,5.2.10);
- analyses the care situation partly on the basis of the family or system-oriented assessment and the conversation with care recipient and family . (1.3.5, 4.1.3);
- uses EBP to analyse the possibilities of information and communication technologies aimed at self-management and co-sufficiency and critically evaluates the use of the chosen technology within the care situation in question. (1.3.5, 4.1.3, 2.2.1, 2.2.3);
- Formulates, in the language of the patient/family, appropriate advice addressed to the family/system based on the assessment, analysis of the care situation and discussion with care recipients and family (including technological options). (1.3.5, 2.1.8, 2.1.9, 5.2.10, 5.2.12).

Content
The Dutch healthcare system is changing; there is a transition from a welfare state to a participation society. Patients are increasingly responsible for their own health, and patients' families and relatives are increasingly expected to participate in caring for a family member in need of care. However, illness and care dependency within a family also affects interpersonal relationships. Families need nurses who jointly guide them towards self-management and co-reliance. Families need nurses who talk to them about mutual expectations, about changing roles, about grief, disappointments, about personal boundaries and about self-care. The balance of giving and receiving is essential for individual family members and for their mutual relationships to sustain care.
In this module, you will demonstrate your ability to engage and work with family members in a responsible and methodical way. You make a professional analysis of a family's/neighbour's care needs, enter into a dialogue with them and eventually write an advice towards self-management and co-reliance in which a good balance in burden and ability to cope for each family member/neighbour is important. In your advice, you include the possibilities within information and communication technologies related to the care situation.
The test in this module consists of an elaboration within the portfolio that incorporates the interpretation , implementation and justification of the learning outcomes. Theoretical substantiation of professional action, reflection and appreciation of practice are compulsory components within this elaboration. This elaboration is the input for the portfolio assessment.
