Learning outcomes
The student:
- outlines the structure and cohesion of the multidisciplinary care process and, on the basis of critical reflection, distinguishes between his/her own (nursing) discipline and other disciplines in a highly complex context (3.4., 6.2.1.);
- formulates how he/she will methodically as well as culturally sensitively arrive at joint decision-making in the coordination of care between patient, his/her relatives and other care providers in a highly complex context (3.1;)
- gives a substantiated opinion of the nursing care provided in a multidisciplinary context and draws conclusions for the continuation of the care process in a highly complex context (3.3.7);
- makes a proposal for improvement of multidisciplinary care provision in a multidisciplinary team in a highly complex context (3.4.3., 3.4.6.);
- uses justifiable, appropriate reporting, transfer instruments, care programmes and/or care pathways in authentic, highly complex, multidisciplinary contexts in transfer situations (3.4.7.);
- demonstrates professional leadership and a proactive attitude when implementing care coordination and applies current laws and regulations in an authentic, high-complex context (6.1.1., 4.1.4., 3.2.6.).
Content
As a student, you are actively coordinating care on a daily basis. In your current work placement period, you also have to do this. In this module, you look for a highly complex care situation in which you see a challenging coordination and continuity task. You describe this critical professional situation and the way you will perform the coordination and continuity task. You then perform these tasks, collect feedback and reflect on your actions.
The emphasis in this assignment is on the execution of your professional actions. The theoretical depth is demonstrated through a poster or a fact sheet (or another form chosen in consultation with the lecturer). The module is tested by means of a portfolio assessment. In this assessment, you justify your choices and show a critical view of your own actions.
The test in this module consists of an elaboration within the portfolio in which the interpretation, execution and justification of the learning outcomes are processed. Theoretical substantiation of professional action, reflection and appreciation of practice are compulsory components within this elaboration. This elaboration is the input for the portfolio assessment.
