Area of Expertise and Learning Outcome(s)

3. Coordinating care – Level 2

LOL 3.1 – RVZ – N2

The novice practitioner works within a complex, authentic context, guided by the principles of person-centred and family-centred care and shared decision-making, and actively seeks, values and integrates the input and involvement of the care recipient and their loved ones in the various phases of the nursing process in order to encourage self-management.

LOL 3.2 - RVZ - N2

The novice practitioner independently prioritises the interests of the care recipient and their loved ones within a complex, authentic context by proactively collaborating intra- and inter-professionally with partners to ensure the continuity and coordination of care.

Content

In this course, the novice practitioner develops the skills needed to ensure continuity of care for their own service user(s). The novice professional learns to apply the nursing role within the healthcare sector, including relevant disciplines and legislation. In doing so, the novice professional works on developing nursing leadership. The novice professional expands their knowledge and skills relating to the coordination of care.

During the sessions, the student gains insight into their own learning process and the steps they must take to achieve the learning outcome. This is achieved, amongst other things, by giving and receiving feedback

Topics covered:

  • Person-centred and family-centred care
  • Shared decision-making
  • Intra- and inter-professional collaboration
  • Proactive care planning
  • Continuity and coordination of care.

Competence Mastery Level 2 (CBN, 2022)

At the second level, the context is highly complex, with the focus on acting methodically in unpredictable, complex situations. The nature of the task is complex and unstructured. The focus is on student-led learning, with guidance provided more on request. Actions are carried out as independently as possible, with justification provided afterwards. The novice professional is encouraged to apply (strategic) planning, self-efficacy, motivation and reflection independently. Reflection on one’s actions centres on daring, willing and being, with the focus on sustainable development.

Assessment:

In this course, students’ competences are assessed on the basis of specific learning outcomes, with the indicators serving as a guide and support for the assessment. The assessment consists of a portfolio assessment. The novice professional determines, in consultation with the lecturer and work supervisor, which supporting documents will be used to demonstrate the learning outcomes. The portfolio assessment is marked using a rubric with a grade and must be passed.
